一度内忧外患、濒临崩溃的堂上综合医院，在外科医生相良浩介（泽村一树 饰）到来的第二个年头，无论从业绩还是口碑均取得了令人满意的成绩。更为重要的是，曾经紧张非常的医患关系得以纾解，一度丧失了责任心的医护人员也受到相良的感召，投入到救死扶伤的工作中去。而就在此时，院长（野际阳子 饰）发表辞职声明，同时指名外甥——相良最大的劲敌森山卓（高岛政伸 饰）担任下任院长。这项人事任免在医院内部引起不小的轰动，人人感到一场强力的风暴即将到来。各种情况的患者接二连三送入堂上医院，相良与团队重逢在救治的第一线，而风头正劲的森山则成为阻挡他的不小的障碍……
